Survey Participation Consent

We are inviting you to be a participant in a research study. This form serves as your consent to participate as a subject in a feedback survey conducted on behalf of the College of Biblical Studies to identify faculty learning styles and technology adoption. This study is conducted by John E. Knight, doctoral candidate for the Ed. D. in Higher Education and Organizational Change from the School of Education and Health Services at Benedictine University.

NOTE: Participation is open to any full-time teaching member of CBS, regardless of computer expertise, age, etc.

• You understand that you will complete a survey with questions having to do with your learning styles and technology adoption, and questions concerning your future intentions to use the digital technologies based on improvements or suggestions that are made regarding its current usability. • You understand that your participation is completely voluntary, and that you are free to withdraw from the study at any time you choose, without penalty. • You understand that this project is not expected to involve risks of harm any greater than those ordinarily encountered in daily life. You also understand that it is not possible to identify all potential risks in any process, but that all reasonable safeguards have been taken to minimize the potential risks. • You understand that the results of this project will be coded in such a way that your identity will not be physically attached to the final data that you produce and that your assigned random tracking number will only be used to track completion of the survey and to prevent duplicate entries in the database on your part. • You understand that the results of this research may be published or reported to government agencies, funding agencies, or scientific groups, but that your name will not be associated in any way with any published results.

This survey will be available to all full-time faculty members of CBS. It will take approximately 20 minutes to complete all forty-one (41) questions.
